The Min­istry of Health is warn­ing the pub­lic, af­ter 300 vials of fen­tanyl were re­port­ed miss­ing fol­low­ing a de­liv­ery dis­crep­an­cy to a pub­lic health fa­cil­i­ty.

The po­tent syn­thet­ic opi­oid, sup­plied as fen­tanyl in­jec­tion 0.05mg/ml/2ml am­poules, was dis­cov­ered un­ac­count­ed for on Au­gust 6.

In a state­ment is­sued yes­ter­day, the Min­istry of Health said it has launched an ur­gent in­ves­ti­ga­tion, and is con­duct­ing site in­spec­tions at dis­patch and de­liv­ery points, ver­i­fy­ing de­liv­ery and stock records, and re­quest­ing re­ports and video footage from the fa­cil­i­ties in­volved.
