The reĀport deĀtailed mulĀtiĀple failĀings that conĀtributed to the acĀciĀdent, inĀcludĀing inĀadĀeĀquate trainĀing, inĀcomĀplete proĀceĀdures, and critĀiĀcal safeĀty checks that were overĀlooked. Ramkissoon, 35, was reĀmovĀing a niĀtroĀgen hose while awaitĀing a steam hose when a fire eruptĀed near the conĀdenser atĀtached to the faĀcilĀiĀtyās fracĀtionĀaĀtion colĀumn.
